Help! How do I make a smooth color transition?

Could someone give me a tip? I just can't seem to get it to look right.

This is what my transitions look like (I complete the last stitch of the old color with the new one):

The easiest way is when you’re on the last stitch in the first color: Just crochet it until you have to pull the yarn through one more time. Pull it through that last time using the new yarn. That way, there’s no overlap between the colors.
P.S.: This works with any crochet stitch, whether it’s a single crochet, double crochet, etc.

That’s exactly how I always do it, too :) Of course, with spiral rounds, you can’t completely avoid seeing the color change, but that’s okay. 

Thanks for the great advice. Sorry, I forgot to mention that I’m talking about color transitions when crocheting in the round. Is there a better way to do this? Mine always ends up looking a little off-center.

The key is to crochet the slip stitch loosely enough so that you can insert the hook cleanly.
