Knitting Pattern: Shawl Collar and Braided Collar.

I’m having trouble following the pattern. I cast on 40 stitches and knitted rows 1–4. I’m having a problem with row 5: when I make a yarn over, slip 1 stitch, knit 3 stitches, and pass the slipped stitch over the 3 knitted stitches, I end up with only 38 stitches on the needle. What am I possibly doing wrong?
